Capstone Project: Strategic Planning Tool

Capstone Project: Strategic Planning Tool

The Capstone Project will initiate change in a healthcare setting.

· It is a field-based planned project within the scope of your current professional role.

· Project scope should be approximately 8 weeks in length.

· Project should be feasible with support of:

· Resources (human, financial, material, and time) Capstone Project: Strategic Planning Tool

· Stakeholders

· Organization/setting

· Project involves application of strategic planning and project management processes.

· Project must address a problem or need.

· Project must be patient-focused and include a goal to improve patient outcomes. Your assessment of the problem and feasibility of a solution must include direct input from patients or recipients of care (survey results, comments, interviews, patient data, etc). Review chapter 22 in the textbook, Leading and Managing in Nursing (Yoder-Wise, 2019) regarding patient-centered care

· Project must include potential, viable solutions. Capstone Project: Strategic Planning Tool

· Project facilitator and setting must be secured with Capstone Project Agreement submission.

Name:

Statement of Need or Statement of Problem to Be Addressed

· Clear, succinct statement of the need/problem that your plan will address

· Two to threeconcise sentences

· Congruent with 8-week scope

EXAMPLE:

The Affordable Care Act (ACA) established the Hospital Readmissions Reduction Program that requires The Centers for Medicare & Medicaid Services (CMS) to reduce payments to hospitals with excess readmissions. The We Do It Best Hospital administration estimates that in 2012, the within 30 days of discharge readmission rate was 40%, resulting in a potential loss of income of $50,000. Background of the Problem

(minimum of three scholarly sources current within the last five years)

· Provide rational and support from literature to validate the need for your capstone project

· Literature support should include scholarly publications, current within five years. Capstone Project: Strategic Planning Tool

· Clear statement of the intended outcome of the capstone project

· Describes the desired “end state”

· Includes SMART characteristics: Specific, Measurable, Agreed-Upon, Realistic outcome-focused statement includingtarget date for achievement.

· Must be patient-focused

EXAMPLE:

Todocument a 2% reduction in the 30-day readmission rateat We Do It Best Hospital by the end of the third quarter (9/30/2015)

· Develop a plan of action to guide implementation your capstone project

· List specific activities that are necessary to achieve the stated project outcome Capstone Project: Strategic Planning Tool

· Each activity must include a timeframe for implementation.

· Activities and timeframes must be congruent with the 8-week scope of the project.

· Must align with the stated project outcome

· Must measure improvement to patient outcomes

· Describe how successful achievement of the project outcome will be determined/ measured (indicator of outcome achievement)

· Describe what type of information must be gathered to validate achievement of the project outcome

EXAMPLE:

Monitor the 30-day readmission rate and compare with rate prior to the capstone project activities in order to validate achievement of 2% reduction in the 30-day readmission rate by 9/30/2015

Within Medilodge, a skilled nursing facility, there lacks an efficient and cohesive medication reconciliation protocol. During the transition of care, patients are vulnerable to medication errors that follow them between healthcare entities. The current medication reconciliation process is often disused by staff due to its timely handwritten nature. Within my plan, I will address the lack of patient and family involvement with medication reconciliation, and foster nursing accountability during the admission process with a new streamlined medication reconciliation protocol. The goal of my strategic plan is to reduce the 30 day readmission rate to hospitals by 25 percent. Capstone Project: Strategic Planning Tool

Statement of Need or Statement of Problem to Be Addressed

Within Medilodge, a skilled nursing facility, there lacks an efficient and thorough process for reviewing and confirming medications with patients, families, and physicians following the transition of care. This deficit highlights the need for a comprehensive protocol reform to be implemented to combat the growing transition of care medication errors and subsequent readmission to hospitals. Capstone Project: Strategic Planning Tool

Background of the Problem

(minimum of three scholarly sources current within the last five years)

“Transition of care from hospital to primary care has been associated with increased medication errors, with an estimate of 60% of medication errors occurring during times of transition” (Kee et al., 2018, p.501).

The process of a medication review consists of reviewing a prior medication list to a current medication list to ensure an accurate list has been generated (Chiewchantanakit et al., 2020).

During the transition of care, healthcare professionals, patients, and their families must undertake the vital medication review process (Hahn-Goldberg et al., 2021). This review period allows patients and their families to review medications and verify that it reflects the current plan of care (Hahn-Goldberg et al., 2021).

This study shows that 16% of the readmissions are caused by medication and that 40% of these are potentially preventable. Most of the medication errors involved in the potentially preventable readmissions were classified as non-adherence (35%) and prescribing errors (35%), followed by transition errors (30%) (Uitvlugt et al., 2021, p.6). 95% of Medilodge nursing staff will adhere to the reformed patient-centered medication reconciliation protocol to increase patient safety and awareness of their treatment plan by the end of October 2022. Formulate and submit an updated medication reconciliation protocol for staff utilizing a three-check method. Similar to the three checks utilized in medication administration.

Verifying medication with reporting nurse

Verifying medications with a second nurse upon entry

Reviewing medications ordered with the patient and or family member.

Verifying medications with physician addressing any discrepancies identified including those by the patient and or family. Capstone Project: Strategic Planning Tool

Obtaining signatures of medications ordered by two nurses and patient (if able) or family member.

Supply patients and or family member (POA/Guardian) with a copy of verified medications.

Obtain approval from facility by September 2nd, 2022. Evaluation will begin with managers reviewing competency sign offs of the implemented inservice educating staff on the new protocol.

Hall managers will monitor admissions for three signatures on medication orders (or two nurses signing off for over the phone reviews with family) during admission reviews.

The goal will be met when 95% of Medilodge licensed nursing staff adhere to the updated medication reconciliation protocol which will be evident through medication order signatures.

Submit a written in-service to management for approval addressing the new medication reconciliation protocol for all licensed nursing staff to read over explaining the new protocol (listed above) and rationale for all admitting patients.

September 6th, 2022
Launch approved inservice to all staff regarding updated medication reconciliation protocol for admissions. September 8th, 2022
Launch updated medication reconciliation protocol within the admission packet. Implement by September 12th, 2022
Monitor compliance of new medication review protocol. By auditing all admissions following implementation of new medication reconciliation protocol for three signatures on medication orders. Providing one-on-one education to those who are noncompliant Capstone Project: Strategic Planning Tool
