Refer back to the interview and evaluation you conducted in the Topic 2 Family Health Assessment assignment. Identify the social determinates of health (SDOH) contributing to the family’s health status. In a 750-1,000 word paper, create a plan of action to incorporate health promotion strategies for this family. Include the following:

  1. Describe the SDOH that affects the family health status. What is the impact of these SDOHs on the family? Discuss why these factors are prevalent in this family.

  2. Based on the information gathered through the family health assessment, recommend age-appropriate screenings for each family member. Provide support and rationale for your suggestions.

Family Health Assessment Part 1

Remember to place to put an introductory paragraph telling your readers why the topic of your paper is important. One or two sentences are good. Be sure to include your thesis statement here. A thesis statement is also called a purpose statement. It would look like this. The purpose of this paper is ( then follow it with a concise statement about what you will be writing about. Family Structure Comment by Lori Jerina: Again, see that the heading is centered and in bold. Note that on this paper it is the second rubric element. That is because the first rubric element ( the questionnaire) will be the Appendix as the last part of the paper as I have shown.

Now you can address this section of the rubric under this heading. According to the rubric, you will want to address the family structure of the family you interviewed. You will want at least one or two paragraphs under this heading. To obtain an excellent you will want to address all the items the rubric suggests ( individuals, attributes, composition-see the rubric!)Family Assessment Paper. Be sure to cite anything that needs to be cited. Remember that if something is paraphrased, is not your words or work, a citation will be needed.

You will now want to address this rubric element with several paragraphs of text. This section asks you to describe the family’s health behaviors and current overall health. Be sure to cite what is needed as well.

Again, now it will be time to address this element of the grading with a few paragraphs of text. This rubric element is asking you to identify two functional health patterns and three health problems as well as any barriers you noted from your assessment. You should demonstrate insight from your assessment findings here as to why you identified them as strength and weaknesses.

Family Systems Theory Comment by Lori Jerina: Again note the last heading and the instructions below

For this section, you will want to address family systems theory and apply it to your family. You will want to show and describe how to use family systems theory to obtain an excellent rating. This means you will show how you plan to use the theory. Be sure to look to the evidence and cite what is needed. Doing so shows you are supporting what you are saying by looking to the evidence.
