The results suggest improvements are needed in the continuity of care of your patients especially when a patient goes into or out of the hospital/emergency department. What plan will you put forward (recommendations) to the provider team and/or the management team? 3. You have a very high number of no shows for appointments. What can you do to improve the number of patients arriving within an acceptable window of time for their appointment?
