Nothing is more concerning in today’s society than the number of deaths that occur daily due to so-called lifestyle disorders. Every day, thousands of new patients are diagnosed, with many uninformed that they are victims of these silent killer ailments. Diabetes, hypertension, and obesity are among the most common lifestyle diseases/conditions (Moghani Lankarani & Assari, 2017). One out of every four Americans has hypertension or is diabetic. These data are concerning, and they have ranked the two conditions among the top ten leading causes of death in the United States. Upon the patient’s first or subsequent visit to a health facility and following the initial patient assessment in triage, the patient will receive a routine checkup that must include the three critical exams of diabetes, hypertension, and BMI. If the patient is diagnosed with one or more conditions, the nurse will access the “Clinic alert follow-up system” (CAFS) and search using the patient’s medical code.
